Wisconsin State law requires dog licenses to be purchased by April 1 of
each year for dogs that are five months of age or older. Licenses are valid
for the calendar year. A “Certificate of Rabies Vaccination” issued by a
veterinarian is required in order to license a dog.

Dog License Application - Mail-In form
Please submit fee, application form and rabies certificate to the
Clerk-Treasurer’s Office, Muskego City Hall, W182 S8200 Racine Avenue,
Post Office Box 749, Muskego, WI 53150 in person or by mail.
You may purchase the license in person during working hours, which are
8:00 AM to 4:30 PM Monday through Friday.
Renewal Dog License - To apply and pay for a dog license renewal only click here (no service charge applies). License and tag will be mailed to you.
If mailed, please include a self-addressed, stamped envelope so license
and tag can be mailed to you.
